To help make your first visit as smooth and efficient as possible, we invite you to download and complete the new patient forms prior to your appointment. Bringing your completed forms with you will allow our team to focus more time on your care and less on paperwork. During your visit, we’ll take detailed records, review your dental history, and answer any questions or concerns you may have. If you experience any issues downloading the forms, simply contact our office and we’ll be happy to email or mail them to you directly.
